Welcome to the new patient center for Southlake Physical Medicine!

We want to make your first visit to Southlake Physical Medicine as easy and comfortable as possible. When you arrive, we'll ask you to complete our patient forms. This will tell us your medical history and information about your condition. If you prefer, you can download and complete the forms at home and bring them with you.

Next, you will have a consultation with Nurse Practitioner Philip Talley to discuss your problems, concerns and possible treatment options. Philip will perform a complete examination to determine the extent and possible cause of the problem. It is possible that further testing will need to be done. X-rays can be taken in our office but if an MRI is necessary, we will make arrangements for you to go to a nearby facility.

Southlake Physical Medicine offers you the incredible advantage of an integrated medical office with the professionals you need working together to diagnose your problem and develop a treatment plan to get you back on your feet and pain free as quickly as possible.

Once the diagnosis is made, all facets of the treatment plan will be explained to you thoroughly and your questions answered. This includes the cost of treatment and the estimated coverage provided by your insurance. We want you to be completely comfortable with the plan that is set forth for your recovery. Then we'll get you set up with a follow up appointment and possible activities to do at home, such as ice, heat or exercises.
